
Patient Background:
Mr. H is a 66-year-old male retired factory worker with a 45-pack-year smoking history who currently smokes 5 cigarettes daily. He has a 5-year history of chronic obstructive pulmonary disease (COPD) and reports worsening exertional dyspnea (mMRC grade 2), chronic productive morning cough, and one moderate COPD exacerbation during the past year requiring oral corticosteroids.
He has been receiving inhaled long-acting muscarinic antagonist (LAMA) monotherapy for 2 years with partial symptom control. Comorbidities include hypertension and benign prostatic hypertrophy managed with tamsulosin. He has no history of asthma or eosinophilia.
Assessment & Diagnosis:

Given persistent dyspnea despite LAMA monotherapy and absence of eosinophilia or asthma features, the care team escalates to once-daily dual bronchodilator therapy using a fixed-dose LAMA/LABA combination administered via a soft mist inhaler. Smoking cessation counseling was reinforced.
